One (1): Full Mouth Set of X-Rays (1 panoramic x-ray - every 5 years)
Two (2): Fluoride Applications (Children)

Plan Details
Plan renewed annually (12 months of benefits begins when you join/pay the membership fee)
Membership fee is due in full at enrollment, is non-refundable, and is renewed annually
All services carry the In-House Dental Warranty Plan
No yearly maximums, no deductibles, and no claim forms
No pre-existing conditions, no pre-authorizations, and no waiting periods
To get the membership discount rate, your portion is due in full on the date of service

Program Exclusions and Limitations
This plan is not a dental insurance plan
This plan does not include nitrous oxide (happy gas), dental products, or whitening products
This plan cannot be used in conjunction with any insurance plan or with any other promotion
